If someone orders a product from the OTO, but cookies have been cleared, then the order will be processed ok but not register in the sales tracking of the database. This is because the system doesn't know who placed the order.
As a result the member won't find the downloaded product when they login to the member panel to pick it up, since the system didn't know who to unlock the product for.
This can happen on our own VSG/PMP sites, since we use VSG/PMP to sell VSG/PMP, or on any customer site.
e.g. this VSG customer experienced it and wrote us the explanation himself:

today, so bought it as soon as it was in paypal,
but my computer is setup to auto remove
cookies and history i did go back to your main page
and in put my email address

nothing came hrough on my emails.
Solution:
- Confirm the order was authorized ok in your payment system (e.g. paypal, swreg, 2checkout etc.)
- Have the person responsible login to the admin panel, go to sales, and create a manual sale for the product for the customer.
- Set commission to auto and the system will identify any referring members and assign them commission.
- Notification emails get sent automatically at this point.
- Inform the customer that they can now login and pickup their purchase download/content from the member panel.
Note that the "commissions for life" feature of VSG/PMP still works and referring members get their commissions, it's just that you need to add the sale manually.
