You can review the standard signup form, and customize it if you want to. You can edit it in merchant panel > Configuration > Affiliate signup
The signup form comes with an information mini-site that contains some information about the affiliate program, etc.
You can edit the text next to signup form, choose what fields should be displayed in the form, set what fields are mandatory or voluntary for affiliates, set your terms & conditions, customize design, set signup commissions, etc. Most of these was already explained in the introduction article.
